After you have answered all the questions, please submit your completed questionnaire to the Alaska Foster Parent Training Center, by pressing the "submit button" at the end of this questionnaire, you may also copy the entire form and paste it into your email program and send it to acrf@nwresource.org.. We will score your answers, credit 1.0 training hour to your training record, and return a scored copy to you. A score of 50% correct or better will entitle you to receive training hours credit. In the event your score is less than 50% correct, you will be able to submit the questionnaire again
